Documents Required for an Immigrant VisaYou must submit the following documents for you and for any accompanyingchildren in advance of your appointment:1. Form DS-260, located at: ceac.state.gov. Submit online.2. Two new photographs matching our Photograph Requirements:3. Valid passport (valid for six months beyond the anticipated interview date).4. Police certificate(s) or Certificate of Non-Conviction: This requirement appliesonly to periods of residence after attaining the age of 16. Applicants living inJordan will submit a certificate of non-conviction instead of a Jordanian PoliceCertificate. Submit a certificate from your country of current residence, ifresidence exceeds six months, and from your country of nationality, if differentfrom the country of current residence and if residence in the country of nationalityexceeds six months. Additionally, a police certificate is required from any countryof previous residence in which an applicant lived for one year of more afterattaining the age of 16. (See Reciprocity by Country.) To obtain a JordanianCertificate of Non-Conviction, applications can be submitted online through theJordanian Government’s official website at www.jordan.gov.jo or by phonethrough the National Call Center: 06-500-8080. Applicants can also obtain thecertificate at the local court (mahkamet al bedaya) in the area where they areresiding. In the absence of a liaison office in the court, applicants should go toPublic Security Directorate office in their area. The certificate is available toresident Jordanian citizens and foreigners regardless of their length of stay.Applicants outside Jordan may approach the Jordanian Embassy in that country forinstructions on obtaining the certificate of non-conviction. This item is required unless your petitioner has alreadysubmitted an I-864 to the National Visa Center.11. Copies of the petitioner's/joint sponsor's federal tax returns (Form IRS-1040)for the same year that the I-864 was filed, and proof of the petitioner’s/jointsponsor’s domicile in the United States. Proof of the petitioner’s/joint sponsor’sassets can also be supplied as supporting documents.12. Arrange for a medical examination to be completed byone of the physicians listed on our Medical Examination page. Please do NOTsubmit the X-ray film, but only the medical reports. If you or the petitioner were previously married, you mustprovide marriage certificates and evidence of termination of all previousmarriages.11. Evidence of support: Evidence that will show that you are not likely to becomedependent upon government support while living in the United States. Generally,you should provide an affidavit of support (Form I-134) from the petitioneraccompanied with a copy of their complete federal income tax returns for the mostrecent year, W-2 forms, and a letter from their employer. You may also provide thefollowing:- A letter from the petitioner’s employer or evidence that they willbe self-supporting;- Evidence of assets available to you in the U.S;- Proof of your educational qualifications and/or vocational skills.12.
